Regular Practicum

The Practicum Office requires candidates enrolled in the BEd/DipEd program to submit the following forms at various deadlines throughout the year.

Assessment Forms

A formative, summative, and self-assessment must be completed for each of the three practicum blocks.  The Practicum Office requires a *copy of the summative assessment only.  The Faculty Liaison requires a copy of each assessment (formative, self, and summative).

*Candidates must keep the original of all assessments.  The original will be needed to make further copies for application purposes. It is very important to keep the original copies in a safe place as the Practicum Office will not make copies from the files.  Make sure both signatures are on all assessments.

Education Placement Agreement Form

Each District School Board requires an Education Placement Agreement form for each candidates working in their schools.  *Candidates will be asked to sign a cumulative list that will be kept on file at Queen's Health and Safety.  Because the Ministry of Education covers the cost of worker's compensation and private insurance coverage, the Ministry may use the information on the Education Placement Agreement form to verify claims.

*The Education Placement Agreement form list will be given to your PROF 190/191 instructor for signature in class.

    On-line Summary of Fall Practicum Form

    Deadline: January 6, 2012.  This form will indicate with whom the candidate worked in the fall term and with whom the candidate will be working upon return to the Associate School in February.

    Intermediate Request Form

    Intermediate-Senior candidates wanting a placement in grade 7/8 for the third teaching block must complete the on-line form by December 9, 2011.  Requests received after this date will not be accepted.  IS candidates must have had highly successful experiences (no checks in Marginal and no more than 5 checks in Satisfactory) in both teaching subjects in the first two teaching blocks in order to be eligible to request grade 7/8.  Grade 7/8 placements are NOT guaranteed, so a tentative placement should be arranged at your original Associate School for the third practicum block.  Normally a placement in a feeder school to your Associate School will be arranged if possible.  Candidates placed in grade 7/8 will be placed as a generalist and must be prepared to teach any subject.  Candidates will not necessarily teach the subjects in which they are being certified.

    Alternative Practicum

    On-line Alternative Practicum Arrangement form

    Deadline:  December 16, 2011.  For candidates who plan to go out of the country, the Canadian North, or to a school in an Ontario School Board for which the Practicum Office must make the request on your behalf (EDU on Board Protocol list) for the Alternative Practicum.

    Deadline:  January 20, 2012.  For candidates in placement settings other than those listed above.

    The Practicum Office will need:

    • Copy of Record of Alternative Practicum Experience form for Alternative practicum
    • Education Placement Agreement - For liability and insurance purposes, Queen's University Environmental Health and Safety department requires all Teacher Candidates to complete and submit an Education Placement Agreement form for the Alternative Practicum.  This form must be signed by all parties and submitted to the Practicum Office, with a copy kept on file at the Host School or Institution.
